Registration Form
   v Please complete the appropriate Religious Education Registration Form (white for weekly sessions;
           green for Home Study).
   v Please enclose tuition payment at the time of registration.
 
Policies
  v   Copies of Baptismal Certificates are required for all NEW students at time of registration (except if baptized at
          Saint Margaret Parish)
   v   Guidelines from the Archdiocese of Baltimore require that a student must have regular attendance in a
          Religious Education Catechesis Program of a Catholic School for one year prior to AND the year making
          each/any sacrament. If your child did not attend our program but attended another program in the year prior
          to a sacrament, please provide a transcript or letter of attendance from that church or school to be eligible to
          receive each/any sacrament this year.
   v   Parents are required to attend seminars before beginning the immediate preparation of their child(ren) for
          sacraments of First Reconciliation, First Eucharist and confirmation. Dates for these seminars will be
          published as well as forwarded to the families via e-mail.
          No one will be denied Religious Education due to financial difficulties. Please contact Mary Ellen Bates,

Contact Information
    v  In an effort to be more efficient in our communication, and take advantage of the technology available to us,
          we are asking that you provide us with a current e-mail address to which we can send notices, announcements,
          confirmations and updates regarding Religious Education. That form is included in this packet.
    v  Please complete the form and return it as soon as possible (but preferably no later than when you register).
    v  All e-mail communication will be sent via "blind copy" in order to ensure the confidentiality of your personal
          e-mail address.
  
Publicity Release Form
    v  During the course of the year, we take many photographs of events and activities. And in most cases,
          children (and sometimes parents and siblings) are in these photographs.
    v  The Publicity Release Form (on the "flip side" of the Contact Information Form) gives us your permission
          to use any of these photographs in our communications and media.
    v  Please complete this form and return it, along with the Contact Information Form.
