These forms are formatted in an industry standard PDF file format and require that you have the Adobe Reader software installed on your computer. The Adobe Reader is a free downloadable software program which is widely used today for electronic document sharing. It is available for

FAITH FORMATION Registration Forms:    
                                   These Forms are required to register children in the various 
                  religious education classes for 2010/2011.

Families should obtain a complete set of forms including introductory  information and guide-lines.  Click here >>>

Be sure to include a Publication Release form with each registration,  and also the New Student/Sacraments form for Students  new to the Program or needing Sacraments.
